As I said somewhere upthread, it’s a combination of Israeli and Arab state policies that have kept the Palestinians from assimilating into any existing state, combined with a policy of rejectionism by the major Palestinian parties that has been a roadblock against the development of a separate Palestinian state. There are even creative solutions like a binational federative state, which would probably have to have the kind of ethnic set-aside offices and parliamentary seats that they have in Lebanon, but there’s simply too much bad blood between the Israelis and Palestinians to go that route.
